Manifesto of Mirai Clinic(mission statement)
We provide best possible care through the goals listed below:
1) We support our patients to continue healthy living.
2) We take our responsibility through actions in order to improve the socio-welfare status of our patients and our staff.
3) We will improve our quality of care by balancing daily practice and medical research.
4) We have close relationships with local hospitals and clinics, the Tama Medical Association, and Tama and Kunitachi municipality.
5) We establish organizational culture in a democratic and bottom-up way.
6) We learn from our patients and the local community by listening to their thoughts and requirements in order to create a better medical practice.
Infomation
Welcome
Mirai Clinic specializes in treating and managing diabetes mellitus. We have three core areas: treatment, education, and research. Our registered diabetes patients have reached 1200 (including 120 type 1 patients), and we regularly see 1000 diabetes patients per month. We have three board-certified dialectologists, two of whom are Fellow of Japanese Society of Internal Medicine, and one is also a Fellow of American College of Physician. We also have large number of Japanese Clinical Diabetes
Educators (CDEJs), and CDEs in West Tokyo area (LCDEs).
We see local patients who have disorders regarding general internal medicine, who seek home-based care, and who require annual health check-ups.
We have sufficient consulting physicians, and each of them is a board-certified specialist in neurology, nephrology, cardiology (non-interventionist), and gastroenterology.
Treatment
Everyone who seeks diabetes care is a candidate at MIrai Clinic.
Treatment is both for newly diagnosed patients and those with diabetes complications.
We have large experience in treating and managing type 1 diabetes patients: from multiple insulin injections to CSII (insulin pump). We are also experienced in treating type 2 diabetes using multiple insulin injections and/or having multiple medical conditions.
We refer patients who need hospitalization to near-by general hospitals.
Education
We are a member of the West Tokyo Diabetes Association (founded in 1999 as Non Profit Organization), and work as task forces at meetings an educational courses.
The West Tokyo Diabetes Association gives accreditation for LCDEs, and we support this activity. At our clinic, we provide diabetes educational courses, gymnastic classes, coking classes to support daily activities of diabetes patients.
Research
In the clinical research, we have been conducting clinical studies especially regarding type 1 patients, and presented at national and international diabetes meetings. Each division in our clinic has its own research plan, and also collaborates with other divisions.

Our mission is to deliver best possible care for diabetes patients. Also, we provide high quality of care in general internal medicine.
Our Main Goals and Objectives
To deliver holistic diabetes care by our staffs as a diabetes team including physicians, nurses, nutritionists, lab technicians, exercise specialists, and administrative staff.
To deliver high quality of care for patients within general internal medicine, we focus on following conditions;

To contribute high standard of clinical diabetes education through various educational courses and meetings and publications in collaborate with the West Tokyo Diabetes Association.
